Medical Benefit vs. Pharmacy Benefit

Here's something really important to understand. There are two ways a drug can be covered: through the medical benefit or the pharmacy benefit. This distinction matters a lot and affects reimbursement, billing, and network requirements.

Medical vs. Pharmacy Benefit Comparison

Billing CodeMedical: J-codes | Pharmacy: NDC numbers
ReimbursementMedical: Usually better | Pharmacy: Often lower
What's CoveredMedical: Drug + nursing + supplies bundled | Pharmacy: Drug only through PBM
NetworkMedical: Provider network | Pharmacy: PBM pharmacy network
⚠️ Critical Warning

A payor can shift a drug from one benefit to the other. When that happens, it can change everything — reimbursement rates, network status, and billing requirements. Always know which benefit a therapy falls under BEFORE you promise a start of care.
